CELL PHONE: With access to a printer,  Open the blank 'Personal Information" form and print or COPY to Email, complete at your convenience.

COMPUTER/IPAD:  Open the form, a fillable PDF, follow the instructions to enter the required information, then print.
or: Print the blank form and complete at your convenience. 

​Bring the completed form with your tax records/documents to your tax appointment. 
Or if you are filing electronically using "Client Exchange"
Upload the completed form using their secure Virtual Tax Portal.
